Futurity Checklist

Nominate bitch after breeding and before whelping date
Download Bitch Nomination Form or nominate online
Complete all lines of Bitch Nomination Form
Mail Bitch Nomination Form with payment and postmark before whelping date
Receive bitch Futurity number within 4 weeks of nomination
Nominate individual puppies after birth and up to 4 months of age
Download Puppy Nomination Form or nominate online.
Complete all lines of Puppy Nomination Form
Mail Puppy Nomination Form with payment and postmark on or before 4 months of age
Receive puppy Futurity number within 4 weeks of nomination
Notify Futurity Chairman of any name change of puppy for the Futurity nomination list sent to the show superintendent
Submit ABC Futurity entry and ABC class entry before show closing date
