Puppy must be nominated on or before four (4) months birthday. Postmark on nomination letter is used as deadline. Please do not send "Return Receipt Requested". If you do not receive acknowledgment within 4 weeks please Contact Futurity chairperson at any of the numbers or the address below.
* Any change of name submitted above must be conveyed to the Futurity chairperson prior to the list being sent to the Superintendent (March 1) or there is great risk that the entry will not be accepted.
**Puppy # will be assigned and copy of this form returned to you as acknowledgment of
nomination.
